Forms

Starting January 22, 2024 all new applications will be subject to the Processes and Procedures Ordinance and must use revised forms (where applicable). These forms are now available at the Department of City Planning's Forms page.

Forms are required for applications that request certain entitlements or administrative clearances. Consult City Planning's Staff Directory or contact one of the Development Services Centers (DSCs) for assistance.

On January 22, 2024 the Processes and Procedures Ordinance will become operative. Projects are not subject to the provisions of this ordinance if a complete application was filed and fees were paid prior to January 22. Prior to this date, applications are required to use the forms listed under the “Chapter 1 Code” column.

All applications filed on or after January 22 will be subject to the Processes & Procedures Ordinance and will be required to use the revised forms under the “Chapter 1A Code” column. The new application forms will not be accepted until the Operative Date, but this will allow applicants to begin preparing their materials in advance and allow additional time for pre-filing questions and consultation.
