To submit a complaint or report a violation of the Restaurant Beverage Program, please follow these step-by-step instructions:
- Go to LADBS "Report a Violation
- Enter the address of the restaurant you would like to report
- Answer the 3 screening questions with "No"
- Provide your name and contact information
- Describe your complaint or violation in the box provided
- Indicate that the property is "Commercial"
- Select "Restaurant Beverage Program" from the drop down menu of complaint types
- Submit
